ACT OF MAY 11, 1912
DECLARATION FOR PENSION

State of TENNESSEE, County of CAMPBELL
 On this 11th day of APRIL, A.D. one thousand nine hundred and sixteen, personally appeared before me, a notary public within  and for the said county and state aforesaid, James Housley, who, being duly sworn according to law, declares that he is 70 years of age and a resident of LaFollette RFD No. 1, county of Campbell, state of Tennessee; and that he is the identical person who was enrolled at Boston Kentucky, under the name of James Housley on the 10th day of March, 1862, as a Private in Co. F 6th Tenn. Infty Vol in the service of the United States, in the Civil war, and was honorably discharged at Nashville Tennessee, on the 24th day of March, 1865. 
That he served no other service.
That he was not employed in the military or naval service of the United States otherwise than as stated above. that his personal description was as follows: height 5 feet 8 inches; complexion, fair; color of eyes blue; color of hair light brown; that his occupation was farmer; that he was born March 16th, 1846, near present residence, Campbell County Tennessee.
That his several places of residence since leaving the service have been as follows: lived in Campbell County Tennessee since discharge
That he is a pensioner under certificate number 545034. 
That he makes this declaration for the purposes of being placed on the pension roll of the United States under the provisions of the act of May 11, 1912.
That his post-office address is LaFollette, RFD No. 1, county of Campbell, State of Tennessee

What was your post office at enlistment? Fincastle Campbell Co. Tenn.

State your wife's full name and her maiden name: Ollie Love Ivey

When, where, and by whom were you married? Near Fincastle by Steven Heatherly J.P.  date August 17th 1885

Is there any official church record of your marriage? County Court record.

If so, where? Jacksboro, Campbell County Tennessee

Were you previously married? If so, state the name of your former wife, the date of the marriage, and the date and place of her death or divorce. If there was more than one previous marriage, let your answer include all previous wives: Catherine Heatherly May the 4 1865 Boy, Campbell County Tennessee May 30th 1884 (Note: her full name was Nancy Catherine Heatherly; her death date is what is listed)

If your present wife was married before her marriage to you. state the name of her former husband, the date of such marriage, and the date and place of his death or divorce, and state whether he ever rendered any military or naval service, and, if so, give name of the organization in which he served. If she was married more than once before her marriage to you, let your answer include all former husbands: Elisha Ivey April the 3d 1881 near Fincastle Campbell County Tennessee. Company F 6 Tennessee Infantry 1st Brigade 2nd Div 23 Army Corps Died October 10th 1883

Respectfully returned to the Commissioner of Pensions.

James Housley, a private of Company F 6 Regiment Tenn. Infty Volunteers, was enrolled on the 10th day of March 1862 at Boston KY 3 years and is reported present upon company rolls from enlistment to Aug. 31 1864. Sept & Oct 1864 not on file. Nov. & Dec. 64 last muster & payroll on file, present. He was promoted to Sergeant Jan 2 64 Company muster-out roll, Nashville Tenn March 24 1865 reports him present and mustered out said date and place. Returns from Nov 62 not on file. No company books or regimental hospital records on file. Records furnish no evidence of disability.

 As qualified administrator of the estate of James Housley (Company F sixth Tennessee regiment U.S.A.) who died on the 17th day of April 1930, I am writing you for information as to the accrued calim of said pensioner from last draw until date of death, including whether or not burial expenses are allowed and all such particulars as are necessary for me to have in order that the matter may be finally disposed of;
The address of James Housley was LaFollette Tenn. My address is Jellico Tennessee, Florence Ave. R.115
