State of Tennessee, County of Perry
On the 11th day of November 1882, personally
appeared before me, Clerk of the Courts, the same being a court of
record within and for County and State aforesaid Betsy Qualls, aged
86 years being a resident of Perry County in the State of
Tennessee, who being duly sworn according to law, declares that she is the
widow of Isham Qualls deceased, who was the identical Isham
Qualls who served under the name Isham Qualls, who served as a
private in the company commanded by Captain (forgotten) in the
Kentucky regiment commanded by (forgotten) in the War of 1812.
That her said husband enlisted and was drafted near
Hopkinsville Kentucky for the term of 3 years and continued
in actual service in said war for the term of 3 years and whose
service terminated by honorable discharge.
She further states that the following is a full
description of her husband at the time of his enlistment, viz: about 24
years old, a farmer, native of South Carolina, about 6 feet tall,
blue eyes, black hair, complexion fair.
She further states that she was married to said
Isham Qualls near (McMinniville?) in the county of Warren
in the State of Tennessee. (dates left blank).
That her name before marriage was Betsy Baucum ; and that she has
not remarried since the death of Isham Qualls. And that she
further states that her said Isham Qualls died at Coon Creek,
near Linden, State of Tennessee, in August, about 1852.
She further declares that the following have been
places of residences of herself and her husband since the date of his
discharge from the Army, viz: near Shelbyville, Bedford County
Tennessee 7 or 8 years; in Lavaca County Texas 7 years; and in Perry
County Tennessee for the remainder of the time.
She makes this declaration for the purpose of
obtaining the Pension to which she may be entitled under Sections 4736 to
4740, inclusive, Revised Statutes, and the Act of March 9, 1878.......
That her residence is Brush Creek, county of
Perry, state of Tennessee, and that her post office address is
Linden, Perry County, Tennessee.

Also personally appeared Alvin Warren, aged 61
years, residing near Linden, in Perry County Tennessee and Enoch Barber
aged 76 years residing near Linden Tennessee.
